Secondary infertility is when a man and woman do not conceive after one year of trying, despite having conceived children in the past without any problems. While primary infertility gets the most media attention, secondary infertility is as common as primary infertility.

According to statistics collected by the Center for Disease Control, 11% of couples who already have a child go on to experience secondary infertility. That’s approximately 4 million families, or about half of all infertility cases.

Unfortunately, because couples experiencing secondary infertility already have a child, their struggle is often downplayed or even ignored by friends, family, couples experiencing primary infertility, and even doctors.

Assuming that infertility “can’t happen” to them, a couple experiencing secondary infertility may delay seeking help for their problems getting pregnant, and wait longer than the recommended 6 months for women over 35, or longer than a year for women younger than 35.

But delaying testing and treatment may mean a lower treatment success rate, as some causes of infertility worsen with time.

Secondary Infertility Causes

Secondary infertility is caused by the same problems that lead to primary infertility. Those causes include:

Male infertility due to low or absent sperm count, problems with sperm shape (also known as sperm morphology), or problems with sperm movement (also known as sperm motility).
Problems with ovulation, whether irregular ovulation or anovulation.
Blocked fallopian tubes.
Endometriosis.
Fibroids.
Recurrent miscarriage.

About one-third of infertility cases are related to male infertility, another third are related to female infertility, and another third are related to problems in both the man and woman, or remain unexplained.

You may be wondering why you’re having trouble this time, when you didn’t have trouble in the past. It’s a good question, but one that unfortunately may not have an answer.

Age may be a factor, especially if you were a lot younger when you had your first child, or you had a late start to your family building. Some causes of infertility may worsen over time, like endometriosis or growing fibroids. If you’ve gained significant weight since your first child, that can also lead to problems conceiving.

But just as often, there are no obvious reasons why you can’t conceive this time as compared to last time.

Testing for Secondary Infertility

Testing for secondary infertility is the same as testing for primary infertility. Both the man and woman need to be checked.

Secondary Infertility Treatments

Treatments for secondary infertility are the same as for primary infertility. Treatments may include:

Fertility drugs, often starting with the most commonly prescribed fertility drug Clomid
IUI
IVF, sometimes in combination with other assisted reproductive options.
Surgery, usually laparoscopic surgery, to repair blocked fallopian tubes, or to remove fibroids or endometriosis deposits

Sometimes when you’re experiencing secondary infertility and need fertility treatment, friends or family may wonder why you are “going to extremes” and insist that since you’ve had a child in the past, you must be able to have one naturally now. If only you “stopped trying”, “relaxed”, “let it happen”, etc.

Be assured that your need for treatment is the same as someone with primary infertility. It is not an “extreme measure” to use medical treatments that may help you have the child you desire.

It is a misconception in majority of the population that if you get pregnant once you have no problem next time.

Secondary infertility is a common problem that can be heartbreaking for many couples. Once you have had a child, you normally think that getting pregnant again should be no problem; after all, haven’t you proven your fertility?

Fertility is a function of age. It starts to decline at age 27, and the most pronounced decline is above age 40.Unfortunately, there’s no test that shows when fertility starts to decline.

Secondary infertility means that couple has at least one biological child but are having difficulty in conceiving for the second time. Secondary infertility may happen at any time and the causes are almost identical to those of primary infertility. The problem can lie with the woman’s reproductive system, the man’s reproductive system, or with both partners.

Many womens dont have any problems getting pregnant for the first time. So they never believe trying for a second child would be an issue. However, even after trying for more then a year if someone is not able to conceive,a visit to the infertility specialists is must.The biggest step in coping with the problem is deciding that a person needs some help this time. A trip to the doctor may result in women undergoing number of tests. The outcome may be secondary infertility.

About 20% of couples who wish to conceive do so within a month. During each subsequent month, about 20% of the remaining couples conceive, so that there is about an 85% chance of conceiving within six months. A physician will generally initiate a medical evaluation only if a couple has not conceived after one year of trying, or, if the woman is over 35, after six months. About one in seven couples have problems conceiving. The percentage of couples experiencing secondary infertility in particular is not known because some couples do not seek medical help.

Why do I have secondary infertility?

There are many reasons why you can have secondary infertility. These reasons are often the same reasons why couples experience primary infertility. See a fertility specialist to try and help diagnose your secondary infertility but some common reasons for secondary infertility include:

* ovulation problems
* uterine fibroids
* endometriosis
* you or your husband/partner got an infection since your last pregnancy
* you or your husband/partner gained weight
* you or your husband/partner changed your diet
* egg or embryo quality has changed
* sperm issues are now present – sperm count, morphology, motility, etc…

There are many reasons you can have secondary infertility. Your lifestyle can be a huge factor in your fertility so check first to see if you have made any lifestyle changes that might be affection your fertility.

There are several factors mentioned below contributing to the Secondary Infertility and which can be controlled by you.

Unavoidable factor in todays life-stress

Stress can be a major factor in trying to get pregnant. The stress hormone cortisol interferes with ovulation, leading to decreased fertility. In addition, stress can also cause elevated blood pressure and other conditions, which can create complications in a pregnancy. Research has shown that 77 percent of women who were able to reduce their levels of cortisol succeeded in restoring their ovulation abilities.

Reducing stress can be done in different ways. Generally, reducing stress hormone levels involves a change in diet and exercising. Meditation has also proved effective as a stress buster. Many doctors may also recommend taking a short vacation to rejuvenate your body.

Fertile period.

The fertile period during the monthly cycle in a women decreases with the increase in the age.Finding fertile period every month becomes a priority because
the egg released the ovary remains fertile only for around 12 hours or so.Ovulation kit or ovulation stick is used to determine when a person is ovulating.

Age and the fertility.

Fertility decreases with the age It is an accepted medical fact that the chances of female not conceiving or infertility in women is higher once she is 30 years old. It is not common with all.In most cases decreased fertility does not hamper a woman’s chances of getting pregnant, especially if she has been pregnant before. However, some women do experience the problem of infertility, termed as secondary infertility. This is more common in women who have had their first child after the age of 30. Many treatments are available to overcome this stage.

Couples under the age of 35 when they fail to conceive after trying for more then a year should consult a specialist and undergo a complete check up.For women over 35, the waiting period is six months.The basic test includes Pap smear test to check for abnormalities in the uterus and ovaries and a three-day FSH/estradiol blood test to measure the level of FSH or follicle-stimulating hormone,which is responsible for release of eggs from the ovary.High levels of the hormone indicate that body is working more to stimulate your ovaries.

What is Infertility?

Infertility is usually defined as the inability to get pregnant after a year of unprotected sex. According to statistics collected by the Centers for Disease Control (CDC), 6.1 million women between the ages of 15 to 44 have an impaired ability to have children, and 2.1 million married couples are experiencing infertility. The statistical study also found that 9.2 million women had made use of infertility services at some time in their life.

Quite clearly, if you and your partner are experiencing infertility, you are not alone.

The definition of infertility can be further broken down into three groups:

Primary Infertility: Primary infertility refers to women who have never achieved pregnancy in the past.
Secondary Infertility: Secondary infertility refers to women who have achieved pregnancy and given birth in the past, but are now having difficulty conceiving.
Recurrent Miscarriage: Women who experience recurrent miscarriages may also receive a diagnosis of infertility if they experience two or more successive miscarriages. While miscarriage is not uncommon (occurring in up to 25% of recognized pregnancies), less than 5% of women will experience two miscarriages in a row, and less than 1% three or more successive miscarriages.
Infertility Myths

This might be a good time to look at some infertility myths, and consider what infertility is not.

Infertility is not limited to women.

Infertility affects women and men equally. According to the American Society of Reproductive Medicine, one-third of infertility cases are due to female factor infertility, one-third are due to male factor infertility, and the remaining third due to problems from both sides, or unexplained reasons.

Infertility is not all in your head.

Infertility is a disease of the reproductive system, and is not caused by not “wanting” to have a baby enough. Infertility can not be imagined into being. If not wanting a baby was enough to cause infertility, then there would be far fewer unintended pregnancies in the world.

Infertility is not limited to unhealthy people.

While living a healthy lifestyle is a good place to start when trying to achieve pregnancy, it does not cure infertility. Poor diet, smoking, drinking, and STDs can threaten your fertility, but the majority of infertility cases are not the result of lifestyle choices.

Infertility is not limited to older couples.

As we age, our ability to achieve pregnancy lowers. Fertility in women peaks during the late teens and 20s, after which it begins to drop, with age 35 beginning the most rapid decline. (This is why couples age 35 and older are encouraged to seek help for infertility after only 6 months of trying.) However, infertility can and does affect men and women of all ages.

Infertility is not going to go away if you just “relax and go on vacation.”

How many times have couples coping with infertility been told, “If you just stop thinking about it, you’ll have a baby.” Not only is this advice incorrect, it’s also hurtful. Extreme stress can disrupt a woman’s menstrual cycle, but stress alone does not cause infertility.

Ignoring infertility does not help, either. While two-thirds of couples seeking infertility treatments will get pregnant and have a baby eventually, couples with diagnosed infertility who do not receive treatment have a 5% or less chance of having a baby.

When is a diagnosis of infertility given?

A diagnosis of infertility is usually given when a couple, after a year of trying to conceive, does not become pregnant.

Another kind of infertility is secondary infertility. Secondary infertility is diagnosed when a couple who has successfully carried to term a pregnancy previously, but now, when trying again, is not able to get pregnant after a year of trying. Also, a couple who suffers from recurrent miscarriages (usually after three successive losses) is also given a diagnosis of infertility.

Is the year wait necessary for an infertility diagnosis?

“Try for a year, and afterwards, come back if you’re not pregnant,” is the most common answer given to couples who are trying to conceive, especially those under the age of 35. But a year is a long time to wait. Do you have to try for a year before seeking help?

Not always.

Look for risk factors or red flags: One way to speed up the process of receiving a diagnosis is to be on the lookout for any risk factors or red flags that may point to an infertility problem. There is no reason to wait an entire year if there are warning signs, symptoms, or past health problems.
Consider charting: Another way to speed up diagnosis is to consider charting your cycles. Charting your cycles involves, at the most basic level, recording your morning temperature, and watching for specific patterns within your cycle.

If you’ve been charting your cycles for six months, and can document to your doctor that you’ve had sexual intercourse around the time of ovulation those six months, and still are not pregnant, they may be more likely to run some tests before your “year” of waiting is up.

If you’re 35 or older, don’t wait more than six months: If you’re over the age of 35, you shouldn’t wait any longer than six months before seeing a doctor. Because your fertility naturally decreases as you age (starting at the age of 25 for men, and at age 30 in women), time is of the essence once you pass age 35.
Who makes the infertility diagnosis?

Usually, your regular gynecologist will make the first diagnosis of infertility. However, that’s not always the case. An endocrinologist or your regular family doctor may alert you to the possibility of fertility problems, if you have health problems that can lead to infertility, like thyroid issues, diabetes, or PCOS.

After the initial diagnosis of infertility, your regular gynecologist may treat you, or you may be referred to a reproductive endocrinologist. Reproductive endocrinologists (RE) specialize in treating infertility in couples.

What causes infertility?

A diagnosis of infertility often comes in two stages, the first stage being the general infertility diagnosis, and the second stage being a diagnosis of the specific cause of the infertility. Infertility is almost a symptom itself. To treat infertility in the best possible way, finding the cause is helpful.

According to the American Society of Reproductive Medicine, one-third of infertility cases are caused by a problem on the woman’s side, one-third are from the man’s side, and the remaining third involve problems on both sides, or unexplained causes. In fact, in 20% of infertility cases, the cause for the infertility remains unknown.

Two of the most common causes of female factor infertility include:

Ovulatory Disorders – accounting for 18% to 30% of infertility in women.
Anatomical Disorders – like blocked fallopian tubes, often the result of infections or inflammations, like endometriosis or pelvic inflammatory disease.

The two most common causes of male factor infertility include:

Low Sperm Production: Medically referred to as azoospermia (complete lack of sperm cells) or oligospermia (few sperm cell are produced), this is the most common cause of male factor infertility.
Sperm Malformations: Even if the amount of sperm is within the normal range, malformed sperm, as well as the ability of the sperm to make it to the egg before dying, can be a cause of infertility.

Whatever the cause is for your infertility, you should know there is reason for hope. According to the American Society of Reproductive Medicine, 85% to 90% of infertility cases can be treated using conventional treatments, like medication or surgical repair. Less than 3% of infertility patients require advanced treatments, like in vitro fertilization.

The issue of secondary infertility is somewhat of a mystery as well as nuisance value to many couples who experience it. Why?

Well, simply because a couple has proven they can already conceive a child yet trying for another and striking a road block such as this can be hard to come to terms with.

What Is Secondary Infertility?

In a nutshell, secondary infertility is being unable to conceive a child after already having had one child. Simply put, a couple will have their first child and let’s say it’s girl. So they then decide to try again for a boy to complete their family. Only problem is, nothing happens.

Despite having proven they are capable of experiencing pregnancy the first time around, the second time is proving a real challenge. What’s the problem? It’s frustrating and in many respects, just as stressful as trying to conceive the first time round.

What Can You Do?

If secondary infertility has been confirmed in your life here are some steps you should consider taking:

- Have a blood test done to check serum progesterone levels as well as levels of serum gonadotrophins and prolactin

- Visit an infertility specialist for diagnosis

- Consider other tests including pelvic ultrasound, hysterosalpingography and laparoscopy

- If the cause has been determined then take the necessary steps to overcome it. You made need to make lifestyle changes such as diet. Cut out things such as alcohol and coffee and refrain from taking unnecessary drug treatments. Even consider alternative treatments.

These are not the be all and end all but just some of the measures you might consider adopting. Your specialist will definitely help you in this regard and point you in the right direction.

Causes Of Secondary Infertility

Causes of secondary infertility can be a “dime a dozen.” Many of the medical conditions namely internal disorders, such as hormonal, ovarian and ovulation problems, tube or peritoneal causes, hypothalamic-pituitary causes, cervical, vaginal and uterine problems, and genetic causes apply here as well. Again, the problem can lie with both the woman’s reproductive system and the man’s reproductive system, or with both partners.

However, most of the times, these are not the only causes. A secondary infertility is commonly caused by age, new partners, stress, taking up alcohol or certain drugs that causes infertility, food, coffee, and many more reasons beyond these. Infertility starts to kick in slowly from age 30 and more rapidly from age 35 onwards in women.

Some women develop hormonal problems which affect the ovulation process, or having not enough mucus to allow the sperm to move freely, or it is too thick and sticky, or the mucus is unreceptive to the partner’s sperm. The mucus, which is supposed to help the sperm to swim up to uterus to meet the egg, fails.

Or it is also possible that a dormant health problem which may have existed when you conceived before but did not affect the pregnancy is active now and it interrupts the current conception. This may include damage to the uterus or fallopian tubes caused by endometriosis, sexually transmitted diseases (STD), fibroids, diseases such diabetes, or any other ovarian or ovulation problems.

Accepting Secondary Infertility

Is there such a thing as secondary infertility. You bet. Secondary infertility is an interesting condition especially when you consider a couple, already with children, suddenly find infertility becoming a problem. So what is secondary infertility and why does it occur?

The question is always asked by couples…” If we were able to have a child first time around, then why is it so difficult this time?” It’s a vexing question and quite legitimate. You can understand a couple’s frustration trying to deal with secondary infertility. So let’s explore some of the possible reasons for secondary infertility.

Examining Family Dynamics

Some of the reasons for secondary infertility can be as simple as not having time or energy for intercourse at your most fertile time of the month because of the demands of your other children.

What About Physical Peak

It could have been several years since you had the first baby and your peak fertile time has passed. Women are their most fertile in their mid-twenties to mid-thirties. Infertility starts casting a greater shadow after 38 and more so after forty.

Significant weight gain by not shedding the pregnancy pounds can also lead to a hormonal imbalance and difficulty in conceiving. Loss of original partner with who the woman had the first child can raise a red flag for reproductive endocrinologists. The new partner may need some additional testing. Other more complicated factors could be polycystic ovarian syndrome, abortions or miscarried pregnancies and uterine or pelvic disorders after the first child was born.

Can Grief Play A Major Part?

One of the emotional rollercoaster rides with secondary infertility is also when couples struggling with primary infertility feel that secondary infertility patients are being ungrateful. ‘At least they have one child’ and ‘At least they have gone through the pregnancy and child birth experience once’ is what they have to say. This can also be a reason why many secondary infertility cases are not talked about.

Couples with secondary infertility issues then seem to play it a little more low key because of their perceived criticism. This is definitely not the way to face up to the problem. For starters, it allows negativity to creep in therefore affecting the state of mind and always in these instances,a positive mind is best. Take it step by step. Allow yourselves a time frame of how long you will keep trying. Consider alternatives if you don’t have success such as adoption. In the end, always prepare yourselves for the fact that it may not happen again for you naturally.
